Please fill out all fields below and click “SUBMIT” at the bottom of the page to complete the sign-up process. Your information will then be sent electronically to CMA and to Hi-Fi Fusion. You will receive an e-mail confirming your submission. Once authorized by CMA, Hi-Fi Fusion will contact you to secure scheduling and the creation of your e-mail/e-card (as applicable).

Please submit a separate Request Form for each e-mail/e-card you want to send.

CMA AWARDS ONLINE VOTING DATES AND INFORMATION

First Ballot will be e-mailed on Monday, Jun 27
with a return deadline of Wednesday, Jul 13 @ 5:00pm CDT

Second Ballot will be e-mailed on Tuesday, Aug 9
with a return deadline of Tuesday, Aug 23 @ 5:00pm CDT

Third/Final Ballot will be e-mailed on Friday, Oct 14 with a return
deadline of Tuesday, Nov 1 @ 5:00pm CDT

If Hi-Fi Fusion is creating your e-mail/e-card, all artwork, specific copy, music, etc. will need to be provided to Hi-Fi Fusion at least two weeks before the desired e-mail/e-card distribution date.

If Hi-Fi Fusion is not creating the e-mail/e-card, all technical requirements will be due to Hi-Fi Fusion a minimum of three business days before the desired e-mail/e-card distribution date.

PAYMENT INFORMATION

Payment will need to be made to CMA in advance of your e-mail/e-card distribution. Check/Money Order should be mailed to Brandi Simms, CMA, One Music Circle South, Nashville, TN 37203. For credit card payments, a CMA representative will contact you.

Hi-Fi Fusion will invoice you once the e-mailing is completed.

CMA Awards E-mail Service Agreement

By submitting this form, I (a.k.a. “Contact Person”) agree to abide by all policies and procedures surrounding the CMA Awards E-mail Service (as outlined here) and pay all charges in connection with this service. I understand that CMA and Hi-Fi Fusion are not responsible for any e-mail distribution delays due to the limitations, inefficiencies and/or security protocols of recipients’ e-mail software and/or e-mail service providers. I also understand that Federal Law guarantees e-mail recipients the right to “unsubscribe” from e-mail distribution lists and accordingly, CMA and Hi-Fi Fusion will provide this “unsubscribe” option on every e-mail distributed through the CMA Awards E-mail Service.
